Study finds type 2 diabetes mechanisms differ by BMI
A recent study published in *Diabetologia* by researchers at Amsterdam UMC and the University of Ghana has identified two distinct biological pathways underlying type 2 diabetes, differentiated by patients’ body mass index (BMI). The investigation shows that in individuals with a higher BMI, the disease is predominantly driven by insulin resistance, whereas in those with a lower BMI range, the primary issue is inadequate insulin production by the pancreas.
The findings suggest that the current one‑size‑fits‑all pharmacological approach to type 2 diabetes may overlook critical differences in disease mechanisms. While insulin‑resistance‑focused therapies such as metformin and GLP‑1 agonists remain standard for many patients, the study highlights that a subset of patients with lower BMI may benefit more from treatments that address insulin deficiency, such as sulfonylureas or insulin therapy. The research underscores the need for personalized medicine strategies that consider BMI‑related pathophysiology.
If these results are confirmed in larger clinical trials, they could prompt a shift toward more individualized treatment regimens, potentially improving glycaemic control and reducing complications for patients whose diabetes is rooted in insulin insufficiency rather than resistance. The study calls for further investigation into tailored therapeutic approaches that align with the underlying metabolic defect in each patient group.
