Air India flight investigation reveals triple hydraulic failure and pilot drug test

India’s Aircraft Accident Investigation Bureau (AAIB) reported that a recent flight experienced a triple‑hydraulic failure, which caused the autopilot to disengage and triggered a stall warning for approximately two seconds. The bureau’s statement indicated that the aircraft’s monitoring systems detected the simultaneous loss of hydraulic pressure across three independent circuits, prompting the automatic shutdown of the autopilot and a brief stall alert.

The AAIB said it is analysing flight data recorders, cockpit voice recordings and aircraft maintenance logs to determine the cause of the hydraulic malfunction and assess whether it contributed to the flight’s trajectory or handling characteristics. Findings from the investigation will be compiled into a safety report, and any recommendations arising from the analysis will be communicated to the operator and relevant aviation authorities to prevent recurrence.
